POLICIES AND PROCEDURES
Waiver/Release of Liability
By registering I agree to:
I, the enrolled participant and/or the parent/guardian of the participant agree and understand that swimming is a HAZARDOUS activity. I recognize that there are risks inherent in the sport of swimming, including but not limited to, paralyzing injuries and death.
The participant hereby agrees to participate in the swimming lessons and hereby agrees to indemnify and hold harmless Blue Dolphin Swim School, its coaches, officers, directors, agents and employees against any liability resulting from any injury that may occur to the participant while participating in the swimming lessons. The participant also agrees to indemnify Blue Dolphin Swim School for any damages incurred arising from any claims, demands, action or cause of action of the participant.
The Participant authorizes any representative of Blue Dolphin Swim School to have the participant treated in any medical emergency during their participation in swimming lessons. Furthermore, the participant and/or parent/guardian agrees to pay all costs associated with medical care and transportation for the participant.
I have noted on this form any medical health problems of which the staff should be aware.
Photography and videotaping is only permitted within the pool area if pre-approved by BDSS Staff. Photography and/or videotaping in the locker/changing room area is STRICTLY PROHIBITED. BDSS reserves the right to deny entry to anyone displaying inappropriate behavior; severe infraction(s) will result in immediate termination of lessons and forfeiture of all monies paid.
I authorize Blue Dolphin Swim School to charge my credit card to cover my student’s initial tuition and, if applicable, the new family one-time registration fee. Pre-payment must be made for the first month of lessons for all new and returning students.
EFFECTIVE OCTOBER 1, 2016:
All current/enrolled BDSS students will be on AutoPay and have authorized BDSS to charge the credit card on file on the 24th of the month for the following month's lessons. VISA and MASTERCARD are accepted. All cards will be charged on the 24th of the month; however, if the 24th falls on a Saturday, your card will be charged a day earlier, on the 23rd. If the 24th falls on a Sunday, your card will be charged a day later, on the 25th. Furthermore, should the 24th fall on a holiday/closure, your card will be charged a day earlier, on the 23rd.
If your card on file comes back as declined, we will contact you immediately. You have until the 1st of the month to pay tuition and update the card on file. A $25 LATE FEE will be charged after the 5th of any month.
Remember, you can update your card on file via the Parent Portal at any time. Please read below on ways to update your account:
1. Access your account via the Parent Portal (on our website); click on payment options, and put in your credit card number.
2. Call the office at 303-254-6920 and give us the information over the phone; we will put the card on file for you.
3. Stop by the front desk when you're here for a swim lesson and have us update your account.
I understand that after registration is finalized with the office of Blue Dolphin Swim School, I have committed to a perpetual swim less/class and am responsible to make monthly payments until I provide Blue Dolphin Swim School with written notice via email by the 15th of the month prior to ending lessons. This is for any cancellation, whether dropping one lesson/class per week or all together. If lessons are discontinued a $10 re-registration fee will be assessed when lessons are resumed. If you cancel after booking and paying for lessons, a $25 fee will be charged to process your refund.
1. Perpetual Lesson.
Our students take lessons until a written cancellation is received via e-mail by our office staff. This is opposed to the session schedule by which Recreation Centers and many other swim programs run. You do not need to call the office to reschedule monthly. You keep your lesson day and time until you change it or cancel it.
2. If my child or I do not like your program, may I cancel before the end of 2 months?
We ask that new families observe lessons before signing up, to be sure the program is the right fit for your child. Once you commit to our program, we commit to your child, and we will do everything we can to help your child to work through any emotional or technical issues they may be having. It takes time to build trust; please allow us the opportunity to establish a relationship and create the comfort needed to build a happy and comfortable swimmer.
3. Procedure to cancel lessons:
Send an e-mail notification to BDSS by the 15th of the month to stop lessons at the end of the month.
4. Make-Up Policy:
We are focused on providing the best swim lesson experience possible. When you enroll your child at BDSS, your lesson time is held exclusively for your child each week. You are paying to secure your child's lesson/class, not their attendance. The consistency of attending your scheduled lesson/class time is one of the keys to our program.
However, we recognize that conflicts do arise occasionally. To address this, BDSS has a make-up policy that has been designed to be both fair and user friendly for the rare occasions when conflicts do arise. The policy is as follows:
Effective February 1st, 2017 we have decided to re-structure our missed lesson policy. As of February 2017 every student may miss a total of 6 lessons, no questions asked. The missed lesson policy operates on a calendar year (December 1st to December 1st). A 24 hour notice is no longer required, however, we still appreciate being informed via email of a missed lesson.
For our BDSS families that have a balance of make-ups from 2016, we are carrying over up to 5 make-ups. The make-ups must be used by June 1st, 2017. This does not affect your ability to participate in our new make-up policy.
As you have in the past, the Parent Portal will be your access to booking make-ups. Remember make-ups need to be used by December 1st and any not used will be voided (they will not roll-over to a new calendar year).
There is NO make-up for a missed make-up.
A credit cannot be issued for a missed lesson/class.
Make-ups are based on space/time availability; teacher requests cannot be taken for make-up lessons. Due to the low instructor/student ratio, we cannot guarantee the availability of a make-up lesson/class.
If you are finding that you must miss your regularly scheduled lesson/class more than on rare occasions, we would be happy to find a new lesson/class at a more convenient time. Please inquire about options with our office staff.
Reminder: Make-ups are based on availability and not guaranteed.
Unplanned Absences are less than 24-hour notice. This is considered a missed lesson, as we must pay our instructor whether you attended your lesson or not. There is no make-up lesson for an unplanned absence.
5. Late for lessons
Our instructors are scheduled in back-to-back lessons and will only be able to take the student for the remainder of the time left in your scheduled lesson time. The instructor will notify the parent of the amount of time left in the lesson. We do not guarantee that any additional time will be available if you are late for your lesson.
6. New or Substitute Teacher
Blue Dolphin Swim School strives for consistency in teachers, however, we cannot guarantee that any student will remain with the same instructor for any amount of time. The consistency in our program lies in the method that we teach, not in the teacher. Obviously, each teacher has their own personality and certain personalities work better with others. If there is a personality conflict, please let the office staff know and the office can assist you with changing the instructor. We do our best to inform our families about teacher changes; however, we do reserve the right to make teacher changes at any time without notice based on company needs.
